This thesis deals with the increase of efficiency of pleural plasty after lung resection in consumptive patients with the usage of developed method of hemithorax correction - pleurocollagenoplasty. It was established that the main cause of complications development after lung resection with hemithorax correction is the formation of a narrow paravertebral cavity, which is not filled with the lung subjected to resection as well as high traumatism of surgical intervention. In the experiment carried out a possibility of application of a collagen sponge for the extra-pleural cavity plombage and mature connective tissue formation at the end of the 2-nd month after surgical intervention was demonstrated. A new method of hemithorax correction based on making of artificial cupula of the pleura after lung resection, preventing displacement of the lung and plombage of the formed extra-pleural cavity with plates of collagen sponge, was worked out and introduced into clinical practice. Introduction into clinical practice of a new method of hemithorax correction made it possible to reduce the time of surgical intervention, to decrease the volume of blood loss and post-operative exudates, to decrease the number of introduced narcotic analgesics, to better functional state of patients in the post-operative period. The developed method of hemithorax correction reduces the number of post-operative complication by 14,6 times, shortened lethality from 5,5% to 0%, increased efficacy of lung resection by 25,9%.
