Vorobey I. Substantiation of surgical tactics in injuries of bowel mesentery as a component of combined injury

The dissertation is devoted to addressing issues related to improving results of diagnosis and treatment of patients with opened and closed abdominal trauma and damage of the bowel mesentery as a component of combined injury. Presented a considerable clinical material, which is based on an analysis of methods of diagnosis, treatment and monitoring of 183 victims with abdominal trauma and damage of the bowel mesentery. Analyzed group of patients with isolated, dominant and competitive intestinal lesions of the mesentery and the mesentery hematoma. Developed and implemented the technique with permanent introduction to the root of mesentery of 0.5% novocaine solution to stimulate intestinal motility and reduce complications in the postoperative period. In the group of patients with combined closed trauma of the abdomen with damage of the bowel mesentery overall mortality was 47.7%, with an open abdominal injury with damage of the bowel mesentery - 13.9%. Developed and implemented protocol circuit diagnostic and therapeutic algorithms of care to victims with abdominal trauma and damage of the bowel mesentery has reduced the time of the preoperative period, the period of postoperative intestinal paresis, reduce complications of traumatic disease.
