Tantsura P. The conservative and surgical treatment of trophic ulcers in patients with chronic vessels disease of the lower extremities

Clinical part of work is based on comprehensive survey of 156 patients with trophic ulcers (TU) of venous etiology, chronic arterial insufficiency of the lower extremities (CAILE), diabetes mеllitus (DM). There were set clinical features of local tissue changes in patients with TU different etiology: 22 (33,3 %) had necrotic; 31 (46,9 %) - dystrophic; 13 (19,8 %) inflammatory type of local degenerative changes. There was developed the program for local treat-ment using the method of local ozone therapy, taking into account the phase of wound healing and character of local degenerative changes in TU zone. It's effectively promoted regression of purulent-inflammatory process and enhance regeneration. To improve the results of skin plasticity of TU based on mathe-matical model method which has been developed, to determine the risk of poor outcome of skin plasticity of patients with TU lower extremities and improve the method autodermoplasty. An algorithm for diagnostic and treatment of TU was developed. The use of the developed treatment allowed for significant in-crease in the frequency of healing TU of venous etiology from 40,4 % to 78,8 % ( p<0,05), with DM - from 45,8 % to 81,2 % ( p<0,05), with CAILE - from 33,3 % to 73,3 % ( p<0,05) cases, and significantly to improve the quality of life of patients.
