Essay

Research topic: the state of androgenization of the organism in male infertility. Research objective: to increase the therapeutic effectiveness as regards the reproductive function disorders in men with idiopathic hypofertility; to meet the above objective the treatment is based on defining some peculiarities in the androgenic status alterations. Methods of research and equipment: clinical, morphological, biochemical, immunofermental, and statistical; a BIOLAN (Russia) reflected-light microscope for photometry; a "STAT-FAX 2100" analyzer, and a KFK-2 electrocolorimeter. A decrease in total testosterone level below 12,0 nmol/l has been established in a third of men with idiopathic pathospermia. Some reduction in free testosterone level and free androgen index values has been revealed in 57,4 and 63,6 % of the analysed groups respectively. It has been also shown that for hypoandrogenic status development in men with idiopathic pathospermia hyporealization of the luteinizing hormone (LH) impact on the Leyding?cells proves to be of vital importance. Hypoandrogenic status development is also related with a certain increase in estradiol and sex hormone binding globulin levels, as well as with testosterone biosynthesis disorders. The treatment patterns stimulating reproductive function have been offered for men with idiopathic infertility depending on pathospermia variant, blood testosterone level, and the index of realization of LH impact on the testicles. The present technologies have been introduced into the work of the room for andrologic investigations and the department of sexual glands pathology of the SI "V. Danilevsky Institute of endocrine pathology problems at the NAMS of Ukraine", as well as into medical institutions of Ukraine (Kyiv, Simferopol, Sumy, Rivne, and Kharkiv).
