Dissertation is devoted to solving urgent scientific task - the development and implementation of differentiated approach to the use of ozone therapy in endoscopic treatment of patients suffering from chronic inflammatory disease of the annexes, into gynecological practice by studying particular mechanisms of its development and effectiveness of treatment aimed at strengthening the prevention of relapses and rehabilitation of reproductive function. The necessity of the use of systemic and local ozone therapy in endoscopic treatment of patients suffering from chronic inflammatory disease of the annexes for the purpose of rehabilitation of reproductive function is proved. It is shown that the use of ozone therapy significantly affects the fibrinolytic system of blood plasma, the structure and intensity of local fibrinolysis, stimulates the functional activity of immune system cellular factors and has a pronounced systemic immunomodulativ effect. The intensity of the proteolytic degradation of high-molecular weight proteins, which provides collagenolytic activity inhibition in the tissues of genitalia, increases. It is shown that concentrations of IL-1beta, TNF-alfa and TGF-beta in plasma are markers of treatment efficacy and clinical course of chronic inflammatory diseases of genitalia when used in multimodal treatment endoscopic and systemic and local ozone therapy. The algorithm for diagnosis and treatment of chronic inflammatory diseases of the annexes, is developed and put into practice, thus reducing the number of relapses and increases the effectiveness of rehabilitation measures.
