The thesis is devoted to the problem of increase the quality of early diagnosis and effective treatment of CHF ischemic patients with left ventricular diastolic dysfunction of the left ventricle based on a study of immune-inflammatory markers, quality of life, and cardiohemodynamics, and their dynamics on the background of standard treatment. The complex evaluation of datas of immune-inflammatory activity, that includes estimation of oxypralin, TNF ?, IL-10, anticardiolipin antibodies levels in patients with CHF of ischemic etiology according to type of diastolic dysfunction was performed. Increased level of oxypralin, TNF ? and anticardiolipin antibodies in patients with CHF with diastolic dysfunction was found, besides the straight correlation between increased level of anticardiolipin antibodies and increased severity of diastolic dysfunction in patients with CHF of ischemic etiology was determinated. The more reliable effectiveness of ARA II candesartan comparing to ACE inhibitor perindopril in therapy of patients with CHF of ischemic etiology because do normalization of hemodynamic datas and increasing of life quality of the patients with CHF according to the type of diastolic dysfunction was found.
