Essay

The dissertation is devoted to the investigation and scientific substantiation of characteristics of clinical course, treatment results and prognosis factors of malignant mesenchymal renal tumors. The paper is based on the data of overall examination and treatment of 114 patients diagnosed with mesenchymal renal tumor and kidney clear-cell cancer. For the first time the investigations performed allowed determining the peculiarities of distribution of the patients with malignant renal tumors by sex and age. It was proved that above-mentioned tumors occur at late stages and possess considerable malignant potential as compared to kidney clear-cell cancer. Analysis of radiation and morphological investigations enabled us to indicate peculiarities of the tumors' vascularization and divide all malignant mesenchymal renal tumors into two groups: "hypovascular" and "hypervascular" tumors. The defined peculiarities of clinical manifestation and factors affecting clinical course of the disease enabled us to develop mathematical model for prognosis of recurrences and metastases among patients with malignant mesenchymal renal tumors. In comparison with kidney clear-cell cancer it was proved that mortality caused by this disease is much higher at the first year after diagnosis and treatment. The data obtained favored the development of practical recommendations for observing the patients of such category in specialized clinic over different post-treatment periods.
