Stepan B. Choice of treatment for patients with pseudocysts of pancreas

The results of treatment were analyzed 358, of which 240 patients with pseudocysts of pancreas were operated. Analysis of clinical picture made possible to define four variants of the clinical course of disease. Sonography with the determination of the echodensity of pseudocyst’s paries and its contents at different stages showed the sensitivity of the method in 93,45 % of cases. Criteria of optimization of surgical policy were justified and algorithm of selecting treatment policy with the preferred choice of miniinvasive methods of the operative treatment of patients with pseudocysts of pancreas was developed considering the assessment of the formation of pseudocysts on the basis of the analysis of sonographic and morphological features on different stages of the formation of cycts. Comparative analysis of surgical policy in different periods of the study showed that the specific gravity of miniinvasive puncture operative interference of patients with pseudocysts of pancreas made 38,33 % moreover for the last period the share of these methods of treatments increased from 1,11 % to 60,67%. Diagnostic and medicative policy of patients with pseudocysts of pancreas which was proposed made possible to reduce the frequency of postoperative complications from 29,73 % when using laparotomical treatments, to 3,26 % when using puncture methods, and postoperative mortality rate from 4,05 % to 0%, respectively.
