The work is devoted to the important task of Cardiology - diagnosis and treatment of atrial fibrillation in hypertensive patients aged on the basis of the nature and severity of myocardial electrical inhomogeneity, determine its relationship to cardiac arrhythmias, structural and functional state of the heart, autonomic regulation, circadian blood pressure dynamics. As a result of the thesis on the basis of clinical and instrumental study the nature and severity of electrical inhomogeneity of atrial and ventricular myocardium in elderly patients with hypertension and atrial fibrillation. It was determined that the late potentials and ventricular fibrillation in the patient groups are associated with an increase in the frequency and amount of daily, respectively, supraventricular and ventricular arrhythmia. Set of structural and functional changes of the heart in elderly patients with hypertension and atrial fibrillation and their relation to electrical inhomogeneity of the myocardium. The character of the dynamics of circadian disturbances in blood pressure, heart rate variability, and a connection to the electrical inhomogeneity of the myocardium. The effect of 3-month treatment aldosterone receptor antagonist spironolactone to noninvasive indices of electrical inhomogeneity myocardium, circadian blood pressure profile in elderly patients with hypertension and concomitant permanent AF.
