Essay

The thesis is focused on reviewing and collecting epidemiological, clinical, morphological and immu-nohistochemical data on dysplastic cervical lesions, which aids in understanding of biological properties of cervical cancer. Also, we review the biological basis for HPV-associated oncogenic transformation of cervical epithelium and advocate the use of immunomodulators in addition to antiviral therapy for the treatment of mild and moderate dysplastic cervical lesions. We suggest the immunophenotype-based schema for diagnosing cervical dysplasia, which can be used as a screening, as well as a confirmatory test for HPV-infection. We also identified the two major immunophenotypic patterns of HPV-associated cervical dysplasia - the so called "regressive" and "progressive" ones. We used the above division as a basis for the differential approach to the treatment of HPV-associated cervical dysplasia, as well as the basis for predicting prognosis and treatment efficacy. We demonstrated the benefits of combined immunomodulating and antiviral treatment with isopri-nosine and interferon, over the standard monotherapy with interferon (p<0,05) in patients with HPV-associated cervical dysplasia of grade I and II.
