Essay

The dissertation focuses on studying hypertensive dyscirculatory encephalopathy course peculiarities, improving its diagnostics by specifying the clinical and paraclinical correlations, based on the data of a complex clinical and instrumental examination. There have been specified the peculiarities of psychological dysfunction indices (the rates of state and trait anxiety) in patients with arterial hypertension. The mean rates of the anxiety were turned out to be increased and the mean rate of trait anxiety was found to be higher in patients whose blood pressure was higher than 160 mm hd, at the same time the proportion of the cases with the high rate of trait anxiety was decreased in the patients who had arterial hypertension more than 10 years as compared with those who had the disease not more than 5 years and from 5 to 10 years. The gender differences as for the indices of state and trait anxiety, namely their evidently higher rates in women were also specified. There was detected a weak direct correlation between the index of the evidence of cognitive disorders (according to MMSE) and arterial hypertension duration (the coefficient of the correlation accounted for +0.34, p<0.01). There was determined a great diagnostic importance of "word material" descriptor with the view of detecting the decrease in the capacity of short-term memory in case of hypertensive dyscirculatory encephalopathy. According to the results of the first correlation analysis of the data of complex rheographic studies of central and regional hemodynamics in patients with hypertensive discirculatory encephalopathy there were detected multiple both direct and indirect correlations of the basic rheoencephalographic, rheorenographic, reovasographic and rheocardiographic parameters which showed polymorphic changes in the blood filling, vascular tone and the venous drainage in the different vascular sections which character was not simple that specified the complex mechanisms of angioregulation.
