Kushnirenko D. Peculiarities of diagnosing and treating unexplained infertility

The research work is devoted to increasing the efficiency of unexplained infertility treatment based on an algorythm of diagnostic, treatment and prevention measures developed and implemented using an improved program of ovulation stimulation. New elements of unexplained infertility nosogenesis have been determined by investigating the relation between the hormonal homeostasis and the ovarian reserve, specific and non-specific immunity, as well as the microbiocenosis of vagina, the cervical channel and urethra. It has been established that unexplained infertility is usually accompanied by compensated microbiological changes, subcompensated immune suppression, and an imbalance of AMH (Anti-M?llerian hormone) and Ttotal (testosterone) levels. The efficiency of improved comprehensive preparation to ovulation stimulation in women with unexplained infertility using anti-estrogens, aromatase inhibitors and recombinant FSH has been proven to increase the pregnancy rate. The data received has enabled scientific justification of the need for improving the ovulation stimulation programs with additional use of aromatase inhibitors.
