Dmytryshyn S. Clinico-organizing model for rendering specialized aid to the patients with urolithiasis.

This work presents a new solution of the actual scientific task - to improve the quality of the specialized aid to the patients with urolithiasis. It bases on the principle of determination of the risk factors in the development of acute inflammatory renal diseases with different variants of treatment, which is one of the main problems in gaining the desirable success. Such an approach allows to prevent complications and, at the same time, to differentially and rationally use the resources and with this giving substantiated preference to the ambulatory-polyclinic and outstationary forms. Such positions ensure medical, social and indirect economic efficacy.The main basis of this work is the results of studying the case records of patients with urolithiasis (508), comments of experts, statistical methods in determination of risk factors about development of acute pyelonephritis in different types of rendering the stationary aid (open surgery, low invasive methods, stenting, nephrostomy substitution , exclusively conservative therapy).By the results of the study there was stated the character of the trend changes in urolithiasis distribution among various types of population in the Ukraine in the regional aspect. The problematic clinical and organizational matters were found out by the quantitative and qualitative signs of the service activity and it was proved that they are characteristic for every region and need improvement. The implementation of the worked out innovations, the vitality of which has been proved, will favour the improvement of the quality of the specialized help to the patients with urolithiasis.
