Essay

The dissertation deals with problems of increasing the efficiency of diagnostics of hemopoiesis disturbances and the course prognosis for chronic glomerulonephritis (CGN) in the 1st stage of chronic kidney disease (CKD) on the basis of investigation of clinical, biochemical, serological, genetic peculiarities of the disease. With the help of a complex examination of children ill with CGN in the 1st stage of CKD, there has been defined the frequency and severity of hemopoiesis disturbances. There has been evaluated the role of hemopoiesis disturbances, in the first place at the expense of cytokine-induced anemia in children in the 1st stage of CKD, their dependence on the form and course of the disease. There have been found out the frequency of iron deficit and disturbances of the indices of ferrokinetics in children ill with CGN in the 1st stage of CKD. On the basis of investigation of individual indices of iron metabolism there has been found their connection with the level of endogenous erythropoietin and the course of the disease. In the process of investigation there has been defined the frequency of combinations of allelic variants of cytokines genes of interleukins - ip and -10 in CGN in children and found the connection of allelic polymorphism with the disease course, indices of hemopoiesis and lL-ip in serum. There has been developed the mathematical model for prognosing the torpid course of CGN in children in disturbances of hemopoiesis and different allelic variants of cytokines genes. In prognosing the level of CGN in children there have been defined the predictors of the disease progressing such as a decrease of the hemoglobin level, erythropoietin, a decreased level of IL-ip in the blood serum and the genotype C/T of the polymorphous portion of the gene IL-ip (-511)
