Essay

There were 144 persons supervised: main group consisting of 102 patients with diabetes mellitus (DM) type 2 and essential hypertension (EH) and control group which consists of 42 patients with EH. Both groups were comparable in gender, age, duration of illness (more than 15 years). Among main group of DM type 2 patients the illness duration exceeded 9 years in average. Among patients with DM type 2 and EH part of diseased persons with effective control of BP is less than in group of patients with EH without diabetes. Body-mass index was found higher in main group. Moreover part of persons with obesity of different level was also higher in main group. High-educated people were found much rarely in group with DM type 2 which is the evidence of more lower social status of patients with comorbide pathology. Less part of married, less part of having job and higher part of disabled were typical for this group of patients. During daily ECG monitoring has been established that patients with DM type 2 in combination with EH in comparison with patients with EH without DM had more frequent segment ST depression and episodes of "silent" miocardial ischemia in relation to all episodes of ischemia during day. In patients with combined pathology ST segment depression during ECG monitoring more frequently did not accompany by considerable increasing of heart rate, and arised later than symptoms of ischemia at ECG more often. The frequency of episodes of "silent" miocardial ischemia rised up with increasing of BP. During investigation of quality of life (QL) of patients of main and control groups there have been established significant differences under the scales of SF-36 questionnaire: BP and GH - in benefit of patients with combined pathology, PF, VT and MH - in benefit of patients with EH without diabetes. During dynamical observation for QL of patients with DM type 2 and EH for 4.5 years in average it has been marked decreasing of possibility to provide their professional activity or home work (pensioners) and deterioration of common level of health in patients with combined pathology.
