The thesis is devoted to choosing the optimal treatment regimen of heart failure in patients with valvular heart defects after prosthetic valve. For this purpose, 100 patients were examined. We conducted analysis of complaints of patients, clinical signs of heart failure, echocardiographic parameters and NT-proBNP level in dynamics was done.It is found that the prosthetic valve provides not only a positive impact on the clinical course of disease, but also promotes regression of signs of heart remodeling. The nature and severity of cardiac remodeling depends on the localization of the prosthesis. Spironolactone was added to the standard treatment regimen of heart failure analysis of dynamics of echocardiography demonstrated that the addition of spironolactone provides a significantly positive effect on patients with aortic valve prosthesis, while, as in prosthetic mitral valve, this pattern isn't traced.Absolute NT-proBNP level indicators of patients with valvular heart disease not informative to determine the severity of heart failure, and only dynamic content biomarker has clinical significance. Increased of NT-proBNP level is associated with over 60 years, female gender and aortic valve prosthesis position.
