Object included 178 patients with lymphedema of extremities in different disease stages; purpose is to improve therapeutic results of patients with lymphedema of extremities by means of extremity volume decrease, decrease of infectious complications level, quality of life improvement by using of low-invasive draining operative resections in conjunction with active rehabilitation; used methods are clinical, instrumental, functional, statistic; novelty is in evaluation of microcirculation state in skin of affected extremities in lymphedema; there were revealed the peculiarities of functional state of integumentary tissue perfusion in different stages of lymphedema; it was established that in lymphedema being a pathologic model of ischemic reperfusion tissue injury is developing regional oxygen deficit and activation of free-radical oxidation processes; it was also ascertained the advantage of liposuction versus classical resection as well as isolated drain operative treatment modes; conclusions: based on the data analysis results of dynamic clinical and instrumental examination the studying of patients quality of life, materials of original developments concerning advanced low-invasive method of surgical therapy of lymphedema of extremities in conjunction with active rehabilitation were established patients hospitalization duration reduce, erysipelas relapse decline, decrease of volume difference of affected and contralateral extremities, and improvement of patients quality of life; is introduced - into medical practice, in educational process; area - medicine.
