Makartsov L. Pathogenetic substantiation of the immune and oxidative disorders correction in patients with severe forms of acute paraproctitis on the background of diabetes mellitus

The dissertation contains the pathogenetic substantiation of the immune and oxidative disorders correction in patients with severe forms of acute paraproctitis and diabetes mellitus using mexidolum and derinat. The features of the immune status of patients with severe forms of acute paraproctitis on the background of diabetes are identified. The role of metabolic disorders in the pathogenesis of acute paraproctitis in patients with diabetes mellitus (activation of lipid peroxidation, antioxidant system failure, development of imbalance in the system of eicosanoids) is established. For the first time the complex effect of mexidolum and derinat aplication on immune and metabolic indices of patients with acute paraproctitis and diabеtes is shown. Two grounded schemes of treatment of patients with acute paraproctitis and diabetes with usage of derіnat (a single intramuscular injection of 75 mg of the drug, previously dissolved by 0.5 % solution of novocain 1:1 and warmed up to a temperature of 35-37 ° C) and mexidolum (intravenous drop injection of 200 mg of 5 % solution twice a day) are proposed. Indications for rational use of derіnat and mexidolum in patients with acute paraproctitis and diabetes: expressive immune and metabolic abnormalities in antioxidant system and in the system of eicosanoids are worked up.
