Essay

The object: secondary post burn glaucoma's. The purpose: to optimize diagnostics and surgical treatment of secondary post burn glaucoma's. Methods: clinical, histomorphological, statistical; apparatus: slit lamp, transpalpebral tonometer, ultrasonic A-B-scan. Performed ultrasonic examinations have shown that the complete obliteration of angle of anterior chamber (AAC) along its perimeter allowed to confirm SPBG occurrence in 100% of observations. Pathological excavation of optic disc was observed in 55,2 % eyes with SPBG. The intraocular pressure (IOP) decrease resulted in decreased excavation level in 27,3% after autoscleral cyclogoniodrainage surgery with autosclera fold (ACGD(F)). Established the possibility to get objective data concerning IOP in patients with scarred changes in anterior segment of the eye with help of transpalpebral tonometer in 99,2 % of cases. Revealed (in experiment) that new antiglaucomatous surgery's hypotensive effect was caused by creation of new ways of aqueous humor outflow from AAC to suprachroidal space, aqueous humor production decreasing in 63,1% as a result of combination of degenerative and atrophic changes in a ciliary body. The IOP compensation was observed in 59,4 % of cases after autoscleral cyclogoniodrainage with strip of autosclera; in 78,4 % of cases after ACGD( F) in 2 years of follow up. The results of researches are implemented in the department of eye burns of State Institution "The Filatov Institute of Eye Diseases and Tissue Therapy of the National Academy of Medical Sciences of Ukraine".
