Essay

The thesis is devoted to the early diagnostics improvement of bronchial asthma formation and progress in children. The significant risk factors for the development of asthma in children with repeated episodes of acute obstructive bronchitis were determined. The connection between asthma severity equivalents and airway hyperresponsiveness level was established. The importance of long-term distal obstruction preservation and bronchial resistance changes not only in relapse but also remission for use as a marker of airway barriers remodeling processes in children with severe asthma was proved. It was indentified the presence of endothelial dysfunction in patients with asthma, which is manifested by increased levels of endothelin-1 in blood and imbalance exchange of nitric oxide metabolites. The pathogenic importance of elevated levels of tumor necrosis factor-alfa and transforming growth factor-beta1 in blood and exhaled air condensate in airway conductance reducing, airway resistance increasing during asthma relapse was revealed. The presence of connective tissue matrix macromolecules disorganization was specified. The importance of communication endothelial dysfunction and disorganization of connective tissue matrix macromolecules in support of the pathological process in the airways.
