Essay

Dissertation is devoted to questions of brain gliomas of different degrees of anaplasia diagnostics and surgical treatment, that is one of most thorny problems of neurooncology and has no generally accepted decision for today. The radial therapy optimization is based on dose distribution forming that considers peritumoral edema form and direction as an area of subclinical distribution of pathological process. At complex treatment while radiotherapy in order to overcome tumor's radioresistance cytostatic chemomedicines were used as radiomodificatiors with the simultaneous brain radioresistance increase due to radioprotector Donovit BC application. On the base of statistic analysis of treatment results of 222 patients with high-grade brain gliomas, main prognostic factors which determine the remote results of radiotherapy and chemoradiotherapy were certain according to index of general life-span, period without relapse, quality of patients' life. At treatment efficacy of patients with brain gliomas estimation according to Kaplan-Meyer method and McDonald criteria it was certain that an account at radial therapy the area of peritumor edema improves for certain treatment results, increasing patient's life-span o the average for 9,4 months, and application of modern chemotherapeutic medicines while chemotherapy in order for radiomodification convincingly testifies expedience of their use in the algorithm of treatment. Chemoradial treatment was most effective in case of temozolomide prescribing at patients with glioblastoma. General efficiency of treatment was 82%, without chemotherapy - just 29%.
