Object - purulent–necrotic (P-N) complications of diabetic foot syndrome (DFS), the purpose - to improve outcomes in patients with DFS by improving the viability of tissue limb gravity P-N lesions DFS and optimization of complex treatment with organ-sparing surgery, and methods - clinical, instrumental, radiographic, hardware, laboratory, bacteriological, cytological, morphological, and math, the statistical; the results - defined the frequency of development, pathogenesis of the course and progression of sepsis in patients with P-N lesions DFS occur when massive affected limb; the first time a new method of determining tissue viability in patients with DFS by measuring the electrical resistance of tissue for the ex-tremities, which allows for early detection of necrotic violations; first proposed a method for determining the severity of P-N lesions DFS, through the definition of the index of aggression, which allows to assess the level of regional endotoxemia and allows generalization to prevent infection and the development of sepsis, first developed by a multi-level diagnostic and treatment algorithm "Diabetic foot", the definition of indications and contraindications to organ-sparing surgery for destructive forms of diabetic foot; branch – medicine.
