Essay

Object: rhegmatogenous retinal detachment. The aim: to optimize the treatment of patients with rhegmatogenous retinal detachment by the use of neuroprotective agents in combination with conventional therapy following surgery on the basis of gaining new scientific data on the role of violations of cytokine expression in the damage of the retina. The methods: The conventional ophthalmic, electro-physiological, immunoenzymometric, and statistical methods of analysis results; slit lamp, ophthalmoscope. The IL-1b, IL-6, IL-4, IL-10 concentration in blood of patients with rhegmatogenous retinal detachment is higher than in healthy individuals. The highest cytokine levels were reveled in the vitreous and subretinal fluids. Disorders of local and general immunity in rhegmatogenous retinal detachment have the same orientation.The relation was established between the direction and the degree of cytokines expression and clinical signs of the disease as follows: the length and height of the detachment, the kind of ruptures, the degree of proliferative vitreoretinopathy, visual acuity, duration of disease. The method of secondary retinal neuroprotection by cortexin has been offered, allowing to receive visual acuity 0,1 or better in 88,6% of patients compared with the control in 2 months after treatment, to save the results in 6 months and also to reduce the cytokines levels in blood in 2 times as compared with the control. The results of the research are implemented in the State Institution "The Filatov Institute of Eye Diseases and Tissue Therapy of the National Academy of Medical Sciences of Ukraine" and "City Medical Center of Eye Microsurgery" № 8, Lviv.
