Object of research: 105 patients with thyroid disease cervical mediastinal localization. A research objective: improving the diagnosis and treatment of thyroid cancer cervical-mediastinal localization complicated compression of the neck and upper mediastinum. Research methods: The clinical and medical history, laboratory, radiation (X-rays, including computed tomography, ultrasound), pnevmotahograficheskie, statistics. Novelty: the first comparative evaluation of diagnostic value of methods to detect mediastinal portion of the thyroid gland and the assessment of the degree of compression complications first developed, statistically validated and clinically tested method of quantifying compression syndrome by CT scan, optimized and clinically tested a method of ultrasound mediastinal portion of the thyroid gland; The effective ways of preventing intra-and postoperative complications. Results: by introducing a new quantitative criteria - the ratio of obturation upper thoracic aperture by CT scan - achieved objective estimation of compression syndrome caused by thyroid disease neck and mediastinal location. Optimization of surgical treatment of this disease has overcome operative mortality and reduce the number of complications from 8.1% to 5.7%, including requiring surgical correction - from 3.6% to 0.9%. Implemented in practice hospitals Donetsk region, in particular the Donetsk Regional Oncology Center, Donetsk city hospital № 1, as well as the pedagogical process of Surgery named K.T.
