The dissertation is devoted to a problem of improving of the results of surgical treatment of patients with destructive pancreatitis in the second stage of disease. Pathophysiological motivation of using of intracorporal EHF irradiation in complex of postoperative treatment of these patients is performed in experimental part because of its bacteriostatic, antioxidant and hypocoagulogic effects. The results of treatment of 113 patients with destructive pancreatitis in stage of purulent complications are performed in clinical part. Dynamic of changing of C-reactive protein's level and leukocytal intoxication level was analyzed and as a matter of record the terms of primary and repeated operations were established. The types of disease's flow were founded on base of changing of ultrasound picture and indications to choosing of certain minimally invasive technology were formulated. Using of staged surgical tactics with primary using of minimally invasive technologies with afteroperative intracorporal EHF irradiation with developed modes allowed to decrease an amount of intra- and postoperative complications from 12,8% to 6% and from 72,9% to 46% accordingly, to decrease an amount of microorganisms with antibiotic resistance from 61,9% to 8%, mortality from 36,5% to 22%.
