The dissertation deals with improvement of methods of diagnostics, prognosis, and treatment of Q-wave myocardial infarction patients in-hospital and postmyocardial infarction period during 12 months. It was shown the role of biological markers of myocardial biomechanical stress that include MMP-3, MMP-9 and NT-pro-BNP as a component of individualized assessment of the risk of unfavorable clinical course and effectiveness of a treatment. It was proved the role of biological markers of myocardial biomechanical stress in unfavorable cardiac remodeling after Q-wave myocardial infarction. It was found the most optimal combinations of biological markers and other factors of unfavorable course of myocardial infarction for improving of their prognostic relevance, sensitivity and specificity. This study has proved the clinical significance of the pleiotropic effect of atorvastatin in high doses that decreases the risk of cardiovascular events including death during 12-month observation period.
