Essay

The thesis is devoted to the optimization of post-operative therapy, aimed at improving the quality of organpreserving surgery in women with uterine leiomyoma, full recovery of the morphology and physiology of the injured myometrium, as well as reproductive function based on the study of morphological features of the structure, blood flow in the area of the regeneration, hemostasis, inflammatory response and cytokine correcting violations. To achieve the goals laboratory, ultrasound, Doppler and statistical methods have been used. Regularities of organ blood flow, uterine perfusion, a pro-inflammatory cytokine response and hemostasis in the dynamics of the reconstruction process in the postoperative regenerate after myomectomy surgery and assessment of their diagnostic and prognostic value have been established at first. It has been shown at first that a prolonged duration of the process of repair due to the persistent violation of organ hemodynamics and blood rheology, that was the basis for the development of early correction of these disorders in the postoperative period. Justification is given for the first time about the start date and duration of use of fibrinolytic therapy in the postoperative period after myomectomy surgery and objective assessment of the therapeutic effect of the combination of streptokinase and streptodornaze on hemodynamics and organ microcirculation perfusion and revascularization, repair processes in the uterine postoperative regenerate.
