Lobova O. Radiofrequency thermoablation in treatment of patients with the cancer tumours of ENT-organs

The dissertation work is directed on the increase of efficiency of surgical treatment of patients with the cancer of larynx of Т1-2N0M0, recurrent and malignant tumours of oropharynx and the laryngeal cancer Т3-4N0M0 with the chemoradioresistant metastases in the thyroid gland on the basis of the developed optimum variant of combination of surgical treatment and radiofrequency thermoablation, intraoperation express-diagnostic of the edge of resection and videoendoscopic monitoring of patients on the different stages of combined treatment. The results of treatment of85 patients with a cancer of larynx of Т1-2N0M0, who underwent the resection of larynx (control group - 42 patients) and laryngeal resection with radiofrequency thermoablation (basic group - 43 patients) were analysed. 25 patients with the recurrent and residual malignant tumours of oropharynx were included into study, in which 12 patients (control group) received cryosurgical treatment and 14 patients (basic group) received RFTA method treatment. Also 9 patients were observed with the laryngeal cancer of Т3-4N0M0 with the chemoradioresistant metastasises in the thyroid gland. Patients with the cancer of larynx of Т1-2N0M0 with the use of local videomicroscopic diagnostic, resection of larynx with the application of radiofrequency abblation, intraoperation express-diagnostic of the edge of resection and videoendoscopic monitoring on the different stages of combined treatment raised ablastic carrying out (organ - keeping) and defined 94,4% barrier of 2-years-old survival-rate without relapse, that on 16% more than in a control group. A total 50% level of "complete response" of radiofrequency abblation in a basic group with the recurrent and remaining malignant tumours of oropharynx was discovered, which is 33,3% higher than in the control group of patients who underwent cryosurgical treatment, which demonstrates the usefulness of RFTA with the purpose of local medical control in this category of patients. In patients with the laryngeal cancer of Т3-4N0M0 with the chemoradioresistant metastases in the thyroid gland it is desirable to perform the surgical removal with the use of abblation by the method of RFTA and histological control of clearness of edge of resection, that allows to get a "complete response" in 44,5% and extends the range of radical surgical treatment in this category of patients.
