Essay

Thesis is devoted to the problem of increasing survival of patients with dilated and ishemic cardiomyopathy in end-stage heart failure who are on the waiting list for cardiac transplantation. The pathogenetically founded algorithm to choose the optimal technique of surgical correction of mitral valve insufficiency in patients with dilated cardiomyopathy is designed. It allows to reach the 100% three-year survival. The optimal timing for cardiac transplantation in patients with dilated cardiomyopathy after correction of mitral valve insufficiency is established. Analysis of long-term results proved the used basic surgical technique of mіtral valve insufficiency repair to be of the most significant influence on patients survival.
