Bakulin P. Kolapsosurgical treatment of patients with destructive pulmonary tuberculosis using thoracic kolagenoplast

The thesis is devoted to efficiency kolapsosurgical treatment of patients with destructive pulmonary tuberculosis by using modified techniques of thoracoplasty and differentiated indications for their use. Developed and introduced into clinical practice the proper technique thoracic kolagenoplast is performance-based on decostation of five upper ribs, extrafascial apicolysis and mediastinal pnevmoliz, fixing the top of the lung, dense tamponade of newly chuck space of the plates of collagen hemostatic sponge, which is the basis for the formation of extrapleural seal. A method of thoracic kolagenoplast is possible to reduce the number of postoperative mortality in 3.2 times, increase efficiency of kolapsosurgical treatment at criteria of termination excretion of bacteria by 27.1%, closing cavities - by 39.6%, to achieve a significant improvement in 81.8% of patients after 2 months after surgery. Application of thoracic kolagenoplast in distant postoperative period led to a 97.5% clinical cure (85.4% - in the comparison group), indicating the high efficiency of the proposed surgical treatment.
