Essay

Aim - to increase the quality of medical care for neonates with pneumonia by improving of diagnosis, objective criteria for the disease monitoring development basing on investigation of inflammation markers in expired air condensate (EAC) and prognosing of pneumonia course. Methods - clinico-anamnestic, biochemical, immunological, statistical. Theoretical and practical results. A device intended for collection of EAC was created, and methodics of its use was developed and approved. Reference levels of nitric oxide (NO2, NO3, NOх), interleukins -6, -8 in EAC were defined in neonates, as well as the dynamics of these metabolites in case of neonatal pneumonia depending on disease period (acute, recovery) and type (ventilator-associated, congenital, nosocomial). It was stated that in all forms of pneumonia levels of nitric oxide metabolites and interleukins-6, -8 elevate. In recowery period, while their level decreases, only level of NO3 is equal to norval values in case of ventilator-associated pneumonia. The role of NO2, NO3, NOх and interleukins-6, -8 in EAC as markers of lung tissue inflammation was postulated. Clinico-paraclinical peculiarities of pneumonia course nowadays were stated. Algorhythm for prognosis of pneumonia prolonged course in neonates with a glance of obstetrical, antropometrical, clinico-laboratory data was developed. This unifies and objectivise the definition of appropriate risk group. Nowelty: the methodics of EAC collection in neonates, defining of inflammation markers in EAC in neonates with pneumonia was developed and approved for the first time. It may be used as prognostic and diagnostic criteria, in development of prognostic algorhythm for prolonged course of pneumonia in neonates. Nowelty of the investigation was confirmed by patent for useful model. Degree of implementation: all acquired results were introduced in pedagogical process at the Department of pediatric propedeutics No. 2 of Kharkiv National Medical University and in clinical practics of hospitals in Kharkiv, Zaporizhzhia, Lviv. Sphere (sector) of implementation - medicine, pediatrics, neonatology.
