Essay

Object of research - alopecia areata. The purpose of research - to improve treatment of alopecia areata and reduce the frequency of relapses dermatosis by developing new pathogenetically substantiated method of combined treatment of alopecia areata including correction of immune status, thiol disulfide system and endogenous intoxication. Methods: clinical, questionnaire-interrogatories, immunological, biochemical, statistics. The thesis covers the problem of increasing the efficacy of the diagnosis and treatment 150 patients of alopecia areata, detection and assessment of its prognostic clinical and laboratory criteria, development of complex method of treatment taking into account clinical and pathogenetic peculiarities of the disease. It is established that the most frequent cause of development or recurrence of the disease is combination of 2-3 unfavourable factors acute or chronic psychoemotional stress being the leading one. Disturbance of T-cell and nonspecific link of immunity, of cytokine level, marked enhancement of the processes of free-radical oxidation of proteins, peroxide oxidation of lipids and development of endogenic intoxication play the leading part in pathogenesis. There were distinguished statistically substantiated criteria of the disease prognosis. The method of differentiated complex therapy of the disease was proposed, which allowed to get good or satisfactory result in 82.5 % of patients. Results have been implemented in the practice of the Odessa Regional dermato-venerological dispensary, the clinic of skin and venereal diseases of Military Medical Clinical Centre of the Southern region, in the pedagogical process of the Department of Dermatology and Venereology of the Odessa National Medical University.
