Object - recurrent and intermittent spontaneous pneumothorax; research - to improve results of treatment in patients with recurrent and intermittent spontaneous pneumothorax by developing a differentiated approach to the choice of advanced approaches and methods of surgical interventions with taking into account of morphological and functional changes of the lungs; methods- instrumental, morphological research, statistical; Novelty - first scientifically substantiated and implemented the algorithm of differential approach to the choice of access and method of surgical intervention taking into account localization and distribution of morphological changes in the lungs of patients with intermittent spontaneous pneumothorax; arguments the use of improved minimally invasive approaches in the treatment of recurrent and cases of the intermittent pneumothorax; the comparative study and justified advantage of the proposed approach of simultaneous surgery with bilateral lesions of the lungs over traditional methods; reasonability of prolonged postoperative conservative therapy with the aim of prevention of acute exacerbation of chronic obstructive pulmonary disease and recurrent pneumothorax; results - introduced complex of medical and preventive solutions can significantly reduce the invasiveness of surgical procedures, reduce the number of complications from 7.6±0.5% to 2.1 ±0.2% in local bullous lung changes, in generalized bullous lung changes from 16.3±0,8% to 4.1±0.4%, reduce the number of postoperative recurrence (when local bullous changes the parenchyma of the lung from 5.1% to 0.8%, in generalized changes parenchyma from 17.2% to 3.3%) in long-term; introduced in medical practice in the teaching process; industry - medicine.
