Yepifantsev V. Optimization of surgical treatment of acute calculous cholangitis and postoperative complications prevention

Object is acute calculous cholangitis; goal is to increase the effectiveness of patients' treatment with acute calculous cholangitis by diagnostic program optimization, multimodality treatment taking into consideration patient's disease severity, prevention of postoperative complications under the control of dynamic interphase tensiometry of blood serum; the methods are clinical, laboratory, instrumental, dynamic interphase tensiometry of blood serum, statistical; the novelty is in the fact that the work includes a new differential approach to the choice of volume and character of surgical therapy of patients with acute calculous cholangitis; results: the implementation of the proposed treatment and diagnostic algorithms permitted to decrease the rates of postoperative complications from 26,6% to 14,7%, lethality rates from 7,7% to 2,9%, therapy term from 14,2 ± 2,3 to 5,9±2,0 days (р<0,05), to improve the quality of life in the long-term period in 93,8% of patients; it is inculcated - in medical practice, in an educational process; sphere - medical practice.
