Essay

The thesis is devoted to solving important scientific and practical problem - improving the effectiveness of treatment in patients with obstructive jaundice, which includes determining the concentration of toxic bile acids in serum, and on this basis to establish the degree of risk of complications and pathogenic choice of surgical treatment. Proved highly predictive and diagnostic power of the method of quantitative determination of bile acids by liquid chromatograph and mass spectrometer, which allows high reliability to evaluate the risk of complications and the severity of the patient with prolonged obstructive jaundice, has high sensitivity and specificity with respect to prediction of hospital mortality in patients well correlated with objective assessment of the severity of the patient's system. The algorithm of medical and surgical treatment in patients with obstructive jaundice, which can promptly determine the most optimal way to correct bile drainage and liver failure, which contributes to improving the effectiveness of treatment, statistically significantly reduced mortality (p ? 0,05), accelerates recovery.
