Essay

Dissertation deals with the problem of respiratory support in patients with severe community-acquired pneumonia (CAP), complicated by the acute respiratory failure. In this study, noninvasive positive pressure ventilation (NIV) was proposed to be used as the first-line treatment of respiratory failure in the mentioned cohort of patients. It was assumed that such approach could potentially improve the outcomes of patients with severe CAP. A number of methods were utilized to examine the epidemiology of NIV in Ukraine, assess the physical status of patients, analyze the parameters of respiratory mechanics, gas exchange and their changes due to performance of NIV, evaluate the outcomes of treatment and compare them between patients receiving NIV and conventional mechanical ventilation. The provided algorithm of NIV in patients with severe CAP, complicated by the acute respiratory failure, based on early application of NIV, individual selection of optimal ventilation parameters and careful monitoring of the patients, reduces the incidence of intubation, the duration of respiratory support, mortality and the incidence of complications.
