Khomenko V. Clinical management of pregnant women at risk of perinatal loss

The aim of the study was to reduce the rate of perinatal morbidity and mortality by improving the management technology of pregnant women at risk of perinatal loss on the basis of comprehensive clinical and laboratory investigation. Was performed the retrospective analysis of clinical records of patients with fetal death to identify risk factors, provided prospective investigation of pregnant women with fetal death to determine some homeostasis peculiarities and developed management technology for pregnant women at risk of perinatal loss. Effect of developed technology on the homeostasis characteristics and its effectiveness was evaluated. Implementation of the technology at the level of large industrial city have lead to reduction of antenatal mortality from 6,3 to 4,98 ‰ (p <0,05), intrapartum death rate from 0,74 to 0,44 ‰ (p<0,05), early neonatal mortality rate from 6,56 to 3,69 ‰. Perinatal mortality rate decreased by 1,6 times (OR 0,64; 95% CI 0,48-0,87).
