Essay

The object of investigation - coronary heart disease (stable exertional angina) of patients with diabetes of 2-nd type. Aim - effieciency improvment of coronary heart disease treatment of patients with type 2 diabetes mellitus under the influence of simvastatin in the traditional therapy and a combination of L-arginine with inosine on its clinical and pathogenetic links, disease course and endothelial dysfunction. It was found that the addition of simvastatin in the traditional therapy and a combination of L-arginine with inosine within 6 months treatment of patients with coronary heart disease with type 2 diabetes mellitus provides a pronounced positive effect on main parameters of coronary heart disease clinical course (average number of angina attacks per day is reduced, also average angina attacks duration and daily dose of nitroglycerin are reduced), body mass index, waist size, hips size are significantly reduced on this background, all parameters of lipid metabolism are normalized (it is reduced the level of total cholesterol, triglycerides, low density lipoproteins, very low density lipoproteins, and also atherogenic index) it is observed a reliable increase of high density lipoproteins after 3 months treatment and a tendency to increase their levels after 6 months treatment, endothelial function is improved (it was increased endothelium dependent vasodilation, it was increased endothelium dependent vasodilation growth, and reduced level of microalbuminuria) also it was noted quality life improvement under all five scales (physical activity, angina stability, angina severity, evaluation of treatment carried on and illness perception) of Seattle Angina Questionnarie. The results were inserted in the hospital practice, scientific and teaching-metodological process. The branch of science in medicine.
