The research work deals with increasing the pregnancy rate based on the development and implementation of an algorhithm of diagnostic, therapeutical and preventive steps using an improved program for controlled ovarian stimulation in women with anovulatory infertility. The work expands our understanding of the pathogenesis of anovulatory infertility by defining the role of immunological and thyroid homeostasis in the development of anovulation. The work justifies the feasibility of studying the dopplerometry indices, which allow to estimate the fertilization capability of a reproductive system, to optimize the pregravid preparation stage before controlled ovarian stimulation, and to increase the overall efficiency of the treatment and preventive complex while reducing the economic load on the patient. The results obtained provide a scientific justification for the need to improve the current schemes for ovarian diagnostics and stimulation in women with anovulatory infertility. The work suggests altering the approach to the diagnostic steps and management of patients with anovulatory infertility towards active, adapted and comprehensive diagnostic and therapeutic preparation to, and conduct of, the controlled ovarian stimulation procedure. The efficiency of the developed comprehensive preparation to controlled ovarian stimulation following a detailed analysis and prediction of the likely results, and with consideration for the dopplerometry indices, has been proved. An increase in the pregnancy rate has been achieved, along with a reduced economic load on patients suffering from anovulatory infertility.
