The thesis is devoted to the study of features of pathogenesis, improvement of diagnosis and treatment of enteral failure in elderly and senile patients with acute diffuse peritonitis. The thesis presents the results of a profound study of the age differences and peculiarities of acute diffuse peritonitis in the application of the priority model and the peculiarities of contaminated redistribution processes in the intestinal-portal pool. The efficiency of the system and regional adjustment during intraoperative intestinal sanitation characterized by a decrease in the concentration of toxic substances and products of lipid peroxidation in v.portae. The introduction into clinical practice of experimental developments of intestinal sanitation during intraoperative and postoperative periods reduced the number of local complications at 13.01%, and overall at 15.69%. Results of surgical treatment of acute diffuse peritonitis in elderly and senile patients of the third group, in contrast to the second group, showed significant advantages of the proposed implementations, thus reducing the number of relaparotomies at 7.95%, and the mortality rate - at 11.6%.
