Study object are complex forms of acute paraproctitises; the study purpose is to improve the results of surgical treatment of patients with complex forms of acute paraproctitises by using of biologically compatible collagen obturator at the expense of quantitative decrease of postoperative complications, decrease of pain syndrome and frequency of relapse appearance; the study methods are general clinical, blood sugar, coagulogramma, biochemical, dynamics studying of local cytokine status, immunological, instrumental, sphincterometry, bacteriological, cytological, visual analog scale (VAS); statistical; the novelty: the studied dynamics of cytological changes of the postoperative wounds at the first time and determined positive reaction of cellular composition of wound іn patients with collagen obturator implantation, a determined negative - by the usage of ligature method, that is in dominance of inflammation type cytograms, it was carried out a comparative analysis of local immunochemical reactions during implementation of traditional treatment methods, it was detected a collagen obturator inertness, that provides an optimal local cytokine balance during the postoperative period of time, it was find out a pro-inflammatory trend of local cytokine status with wound process prolongation by the usage of ligature method; the result is determined high efficacy of the proposed method that is explained by the absence of anal sphincter damage during the operation and by the absence of scarry stricture of anal canal because of the optimal behavior of wounding process; аll the methods are introduced in a medical practice, educational process;branch-medicine.
