Essay

The dissertation is devoted to studying the role of immunological, morphological and molecular genetic predictors in the development of nasal polyps (NP) and its recurrence. We used the following methods of examination: clinical, immunological, morphological molecular and genetics. Patients with NP characterized with increasing phagocytic index of neutrophils, their oxidative "stress", CD3+/CD HLA-DR+-, CD4+CD25+ FoxP3+- lymphocytes simultaneously with a decrease of the number of NK cells and phagocytic index of monocytes compared with patients with allergic rhinitis. Monocytes more actively expressed TLR4 in the conditions of colonization of Staphylococcus aureus on the nasal mucosa and TLR9 - conditions for reactivation of EBV infection in the lining of the nose in patients with NP. Type of allelic polymorphism of LMP 2 His-His in patients with NP accompanied by higher rates of phagocytosis of neutrophils and activated T lymphocytes, lower rates of phagocytosis of monocytes and the number of CD16+/56+ lymphocytes, compared with patients with homozygous type PRS Arg-Arg. Eosinifilic type of polypos were characterized by a decrease in phagocytic index monocytes, increasing their oxidative "stress" for physiological stimulation, increased number of CD3+/CD HLA-DR+- and reduce of CD16+56+ lymphocytes, and the prevalence of hyper IgE-syndrome, enhanced replication of EBV infection in combination with Staphylococcus aureus colonization and increased expression of miRNA 125b. Based on logistic regression analysis, established risk factors for recurrence of NP.
