The thesis is devoted to solving current issues of modern obstetrics and gynecology - reducing the rate of placental dysfunction of pregnant women with a high level of infectious risk, based on researches of clinical and laboratory, endocrinology, immunology, functional and morphological characteristics of the functional state of the fetoplacental complex and genital tract microbiocenosis, as well as the development and implementation of the improved algorithm of forecasting and preventive measures for these pregnant women. The author has studied the features of the formation of fetoplacental complex, pregnancy, childbirth, fetus and newborn condition, identified the risk factors for placental dysfunction and its degree of complication for pregnant women with a high level of infectious risk. Based on a comprehensive study of the fetoplacental complex new additional data about the features of placental dysfunction of pregnant women with a high level of infectious risk, hormonal function, morphology and histology of placenta and the uteroplacental-fetal circulation, fetal development and newborn condition were obtained. The features of systemic and local immunity were found, diagnostic and prognostic value of levels of cytokine profile of pregnant women with a high level of infectious risk relative to placental insufficiency of varying severity were proven. This made it possible to extend the existing data on the pathogenesis of placental dysfunction of infectious origin and to ground scientifically the necessity of prognostic-preventive-treatment measures to reduce the frequency and severity of placental dysfunction of infectious origin. The author has created the examination algorithm for pregnant women with a high level of infectious risk, improved methods of prediction and prevention of placental dysfunction, which made it possible to reduce the rate of obstetric and perinatal complications in this group of pregnant women.
