Object of the study:Object of the study: cardiovascular system of patients with diabetes mellitus (DM) type 1. Aim of the stadi: to examine the functional status of intracardiac hemodynamics and vascular endothelium in young patients with type 1 diabetes, identify the characteristics of metabolic and immune disorders and establish early diagnostic criteria of metabolic cardiomyopathy. Research methods and techniques: general clinical, biochemical, functional , immunological, ELISA and statistics; ultrasound machine «ALOKA- 5000", the sensor frequency of 7.5 MHz. Theoretical and practical results: a comprehensive study of patients with type 1 diabetes showed 65,7 % of patients have subclinical signs of diastolic dysfunction of the heart, which is a marker of early myocardial injury, systolic dysfunction and heart failure. The relationship of intracardiac hemodynamic changes of endothelial dysfunction , which is a component in the development of cardiomyopathy was presented. Exploring the metabolic and immunological parameters established their relationship in violation of endothelial function and the development of diastolic dysfunction, diagnosis and treatment of which will prevent the progression of cardiomyopathy. Novelty: the firstly to show that patients with diabetes type 1 diabetic cardiomyopathy is accompanied by the immune system with increased levels of proinflammatory as ( lL -6 and TNF -?), and anti-inflammatory ( lL -10) cytokines. These data suggest that the violation of diastolic myocardial function observed in type 1 diabetes duration of more than 5 years. It was established that in patients with type 1 diabetes endothelial dysfunction is one of the factors involved in the development of diastolic dysfunction of the heart.
