Vyzhga Y. Clinical-immunological peculiarities of the juvenile rheumatoid arthritis and estimation of the basic treatment effect

The dissertation is devoted to the questions of the pathogenesis, clinical currency and estimation of the basic treatment effect while evaluation of the clinical-anamnestic, biochemical, immune points in children with JRA. With a help of complex study of the children we estimated peculiarities of the clinical currency of the disease, stage of the functional insufficiency in dependence on term of the disease. We found peculiarities of the immune-inflammatory response due to cascade activation of the transcriptional factor NF-kB. We proved actuality of the estimation of the anti-MCV in serum like a diagnostic marker of JRA, analyzed its connection with other signs of the immune response and clinical manifestation of the disease. We estimated peculiarities of the content of the inflammatory cytokines – interleukin-1? and -6 in children with different duration of the disease, so we found that at the beginning of the disease and during the first 2-years we observe the prevalence of the IL-1? increasing. We checked effect of the complex drug therapy of the JRA with methotrexate, sulfasalazine and leflunomide administration according to the ACR criteria and dynamics of the main immune-inflammatory markers. We found that depression of the immune-inflammatory aggression and transcriptional factors of the cellular nuclear is achieved in a full volume under the therapy of methotrexate and leflunomide, but saved effect of the basic treatment can be achieved just with methotrexate administration. .
