The object of study - chronic heart failure I-IV NYHA.It was found that endogenous intoxication syndrome is seen in virtually all patients with CHF that adversely affects the course of the disease manifested by increased severity of CHF, renal impairment, compromised liver function, maladaptation, and more significant abnormalities of cardiac structure and function. Taking into account the severity of endogenous intoxication syndrome and grade of obesity, the adverse prognostic predictors were established that can be used in clinical practice. Based on the results, the program for calculation of absolute risk of adverse CHF clinical outcome was proposed. It was shown that blood level of the average weight molecules, the type of the general nonspecific adaptive reactions, serum cholesterol level, blood L-arginine level, the urine nitrites, and glomerular filtration rate were simple informative criteria for patients with CHF. Thus, abnormal results of these parameters indicate patients with a high probability of unfavorable clinical outcome that is important for optimization of CHF treatment options and strategies.
