Essay

The dissertation is devoted to the improvement of individual and differential approach to the rational choice of treatment in patients with acquired strictures of various etiologies (iatrogenic and due to a long stay in the ureter concretion) and congenital strictures (obliteration) of the ureter using endoscopy (Ho-Yag laser, bougienage with plastic urethral pathfinder and "cold" knife) restoration of patency of the ureter. In the dissertation are presented the results of examination and treatment 118 patients with acquired and congenital strictures (obliteration) of the ureter. A separate group includes patients with ureteral strictures that emerged as a result of the imposition on the wall of the ureter suture material that is not resorbed. For endoscopic restoration of patency UUT (upper urinary tracts) at presence of acquired and congenital stricture of ureter we applied three methods: laser ureterotomiya, endoscopic bougienage of strictures and dissection them using "cold" knife. Proved that at the moment of operation, 100% effect was observed in those patients which were used Ho-Yag laser. Some what lower efficiency - 80% - was observed in patients with congenitalureteral strictures treated using plastic ureteral pathfinder and "cold" knife. Should be noted, that ineffective surgery in the two groups of patients occurred in those patients, where the length of the stricture was 0.5 to 1.5 cm.Developed an algorithm for treatment of patients with ureteral strictures of various etiologies and indications for endoscopic recanalization established the basic factors of acquired and congenital of strictures UUP. Is proved high efficiency of X-ray-endourological treatment of UUT (upper urinary tracts) strictures using Ho-Yag laser that allows to use it as an alternative to open surgical treatment. Clinical effect of using Ho-Yag laser under ureter strictures to 3 cm is 80%.
