Essay

The thesis deals with the issues of clinical features of chronic laryngitis in patients with persistent herpes virus infection, the study of the immunological status and some interleukins blood serum of these patients. Relevance of the topic due to the prevalence of chronic inflammatory diseases of the larynx and the poor results of treatment of these patients (chronic laryngitis is considered by clinicians as a precancerous condition). Analyzed the frequency of detection of microbial pathogens in hospitalized patients with CL in accordance with the data of microbiological, larynx microscopy examination of these patients. The frequency of persistent herpes virus infection (HVI), which is considered by many experts as an opportunistic infection, the human herpes simplex virus type - (HSV) leads to the formation of immune suppression, primarily cellular immunity and disturbance in the "cytokine setiі." In particular, there is a difference in the cytokine profile of patients with the activation of persistent CL ? HSV type without it. In patients with the activation of persistent CL ? type HSV infection were 2 types of serum response: 1-a group (I-A), in which patients had increased all studied interleukins, so-called "cytokine burst", which indicates the serious nature of the flow of the inflammatory process and requires intensive anti-inflammatory and detoxification therapy, group 2 (I-B) with an imbalance of the cytokine profile: reduction of gama--IFN and TNF-alfa accompanied by increased IL-1 and IL-6. determination of cytokine mediators of cell-cell interactions of the spectrum has prognostic significance: the definition of a specific patient CL with persistent infection with HSV type - increasing levels of IL-6 in two or more times on the background of reduced levels of --IFN and TNF- is a prognostic unfavorable situation that precedes the parts recurrence of inflammation of the larynx (as Catamnesis). In the group of I-A, which was an increase of all investigated interleukins, so-called "cytokine burst", in the first 5 days shows the assignment of systemic glucocorticoids, and on the 6th day of immunomodulator Erbisol according to our methodology. In group 1-B (higher levels of IL-6 on the background reduction gama--IFN and TNF- shows replacement therapy with interferon, and a 6-day introduction to interferonogen Cycloferon at epiglottis space. Held diagnostics and differential treatment of patients of CL with persistent HVI. In their detailed analysis of the evidence of their high efficiency compared to a comparison group. In these patients at follow-up, however, as one year reduced the number of exacerbations CL .
