Essay

The thesis covers the issues of establishment of new pathogenesis-based methods and means for the local treatment of necrotic suppurative wounds and trophic ulcers of the lower extremities. There has been developed and proposed a number of dressings with antibacterial and reparative properties and the offloading orthosis for the local treatment of necrotic suppurative wounds of the soft tissues of the lower extremities. The use of these materials and means in clinical practice can significantly reduce the average duration of treatment, increase bearing capacity of the extremity, mobility of patients, and improve their emotional state. It has been morphologically proved that the increase in the index of the macrophagal activity in the treatment of necrotic suppurative wounds and trophic ulcers is a reliable criterion for the effectiveness of complex treatment using the suggested surgical dressings. In addition, monitoring of the indicators of endothelial dysfunction, hemostatic system, markers of the inflammatory response, and the indicator of autoimmune processes correlates with the clinical manifestations and the course of wound healing and indicates the effectiveness of the proposed methods and means.
