This scientific research studies clinical and pathogenesis features and quality improvement of diagnostics and treatment efficacy of reactive arthritis with concomitant CKD I-II stages.: Pyelonephritis on the basis of the study of inflammatory parameters, blood cytokine spectrum, components condition of the connective tissue, kidney function. Chronic pyelonephritis likely impact on the increase of RA degree of activity was determined; facilitates its advance in terms of connective tissue inflammation and reaction. Among patients with RA and comorbidity of CKD renal injury at the level of tubular and glomerulose vessels was diagnosed, what occurred within increase of fibronectin and nitrituria, recession of creatinine level in urine, GFR and significant urinary syndrome. Significant increase in blood of free oxyproline, bound oxyproline, collagenoses intensity, proteolysis of low molecular weight proteins was determined among patients with RA and comorbidity of CKD:CP compared to isolated progress of RA. Patients with RA and comorbidity of CKD:CP within the chronic arthritis had increase of IL-6, IL-1beta, IF-gama and reduction of anti-inflammatory IL-4, and with relapsing course, the increase in blood levels of anti-inflammatory cytokines IL-4 and IL-1Ra and proinflammatory FNP-alfa. Administration of аntibodies to human tumor necrosis factor-alfa decreased activity of іmmunoinflammatory process, the improvement of renal function and quality of patients’ lives.
